Schedule a Date/Time For The SAP Evaluation
During the engagement, the Substance Abuse Professional (SAP) will complete a clinical assessment to get necessary information pertaining to the reason for your offense.

Complete Evaluation to Outline “Return to Duty” Plan
Your Substance Abuse Professional (SAP) will review your assessment and determine the most appropriate educational program or treatment for your treatment plan. The goal is to gain the knowledge and skills to reduce a repeated offense.

Send A SAP Request
- Select a verified DOT Substance Abuse Professional (SAP Evaluations, LLC)
2. Contact the Substance Abuse Professional. Your Substance Abuse Professional (SAP) will accept your request.
YOU NOW HAVE A DESIGNATED SAP PROVIDER
3. Schedule your SAP assessment face to face (virtually)
4. Once the assessment is complete, the SAP initial evaluation report will be uploaded into FMCSA Clearinghouse
5. Your SAP will make recommendations that are specific to your needs. The treatment plan will be completed.
It can include education, treatment or etc.
4. A follow up evaluation will be conducted. You must comply with your recommendations in order to receive a RETURN TO DUTY letter. Then you can become eligible to participate in RETURN TO DUTY TESTING with your current or prospective employer.
5. If you receive a NEGATIVE drug test, you are not eligible to return back to your safety sensitive position.

My name is Shaleatha Barnes. I am a graduate of George Mason University in Fairfax, Virgina. I received a Master’s in Social Work from this institution. I am a licensed SAP (Substance Abuse Professional), and licensed Social Worker in Maryland and District of Columbia. I have a history working with clients who have substance abuse issues, mental health disorders and have other relevant barriers that inhibit their functioning. I have worked in this field for several years establishing treatment plans that are client centered that meet the unique needs of my clients.

How Long Does SAP Evaluations take?
SAP evaluations take about 45 minutes to complete. A virtual face to face call will be used to complete the evaluation.
How much are your fees for SAP evaluations?
The SAP Evaluations are $350 (This includes the Assessment, Initial SAP Evaluation Report, and Final Evaluation Report)
What is the FMSCA?
Is an online database that provides information that is used to improve the safety of the highway and the drivers. It helps to enforce DOT regulations. (SAP Evaluators, Drug Test Results, SAP Evaluations, Return to Duty process, and the Follow up Testing Plans are found on the FMCSA clearinghouse)
What are common recommendations for the treatment plans?
Substance Abuse Classes, Alcohol and Drug Awareness Classes, ASAM Level 1 Treatment, Individual Therapy or other relevant recommendations based on client’s specific needs.
